BIRTH CERTIFICATES FOR OKALOOSA COUNTY

HOW TO OBTAIN CERTIFICATIONS OF BIRTH FOR OKALOOSA AND ALL OTHER COUNTIES IN THE 
STATE AND DEATH CERTIFICATIONS THAT OCCUR IN OKALOOSA COUNTY.

BIRTHS

The birth you are concerned with must have occurred in Florida

You must explain your relationship to the person whose record you are 
requesting.
	By law birth records can only be issued to the person to whom the record 
is filed, if at least 18 years 	old, or a parent, guardian or legal 
representative or upon court order.

The fee for a birth record for a person born in Okaloosa County, $10.00 first 
copy, $5.00 each additional certification of the same record when ordered at the 
same time.

Vital Statistics is able to generate births that occur in any of the counties in 
the State, i.e. Escambia, Walton, Santa Rosa, etc. dating back to 1962.

The fee for a birth record for a person born outside Okaloosa County, is $11.00 
first copy, $7.00 each additional certification of the same record when ordered 
at the same time.

TO ORDER A BIRTH CERTIFICATE BY EITHER MAIL OR FAX, PROVIDE A SIGNED LETTER 
WHICH INCLUDES THE FOLLOWING INFORMATION:

	1.	Information about the person whose record is being requested: 
Full name at birth and any 		subsequent name change that may have 
affected the indexing status of the birth records, 		date of birth, 
county of birth, father's name and mother's full name, including her maiden 	
	surname.

	
	2.	Information about the person making the request;
		full name, relationship to the person named on the record, 
mailing address, telephone 		number where you can be reached during 
our office hours.

	3.	Send a money order made payable to the "Office of Vital 
Statistics."

American Express.  There is an additional charge for this service for shipping

DEATHS

The death must have occurred in Florida.

If you are requesting the cause of death, you must explain your relationship to 
the person whose record you are requesting.  Death's that need to be recorded in 
a court house should be requested without cause of death.

The fee for a death record is $7.00 for one certificate and $7.00 each 
additional certification ordered at the same time.

Fee are non-refundable.

To order a death certificate by mail or fax, provide a signed letter which 
includes the following information:

1.	Information about the person whose record is being requested;
	Full name of the deceased person
	Sex
	Date of death
	County of death

2.	Information about the person making the request;
	Full name
	Relationship to the decedent, if cause of death is needed
	Mailing address
	Telephone number where you can be reached during office hours

3.	Send money order made payable to Office of Vital Statistics

If you are not sure of the date of death provide a range of years to be 
searched.  An additional fee of $3.00 each additional year is required.
